Directions for the Application for Enrollment. (You may also utilize our online registration.)

1. Print out all four pages.

2. Complete and return pages one and two.

3. Be sure to sign the application on page two.

4. Be sure to complete the credit card information or provide a check or money order for the $200/child non-refundable, non-transferable deposit. We cannot hold a space for your child until we receive a deposit. We will not process your deposit if the desired session is full.

5. Review and retain the policies on pages three and four of the application.

The necessary paperwork for camp is provided below. These documents are available in AdobeĀ® .pdf format. These documents can be mailed to Camp St. Charles, 9692 Meadowview Drive, Newburg MD 20664 or faxed to 301-576-5944.

DO NOT FAX HEALTH FORMS!!An original signature is required, the first page of the form MUST not be faxed. If you faxed the form, you will have to fill out the first page of the form again at check in.

Every camper must have a health form on file, it is required by the state of Maryland. Any child who take medication or has a medical condition (asthma, allergies, heart condition, etc) must have a physical exam that is less than one year old. Any camper with no medications and no medical conditions may use a physical that is less than two years old. We recommend an annual physical for all campers. If your child attended camp last year and does not need a new physical exam, you still need to complete pages 1-3 of the health form so that we have permission to provide emergency treatment/transportation if necessary. Emergency rooms, dr's offices, etc will not treat your child without this permission unless it is a life threatening situation. We will contact parents if we need to seek treatment, but parents are not always available. If you plan to use a physical from last summer or any other source, you may attach the physical to the camp health form. Contact us if you need to check the date of your child's physical.

Why do parents need to complete the camp health form if they already had a physical for another reason (sports, school, another camp)? In an emergency, we need to have instant access to your child's health history. If we are searching an unfamiliar form, it wastes critcal time in an emergency. We want to be prepared to provide the best possible care for your child.
